My name is Sophie Chapman from SRI International, Menlo Park campus in California. I will be issuing the PO to IPOL for the travel/transportation services. Before releasing the PO, I have a few questions I hope you can clarify…

1. The attached quote (estimate sheet) states a deposit is required before customer’s arrival. Is the $4,155 the deposit amount only? Or is it the full amount for the period from March 7, 2012 to Mary 13, 2012?



2. I am assuming the above amount of $4,155 US Dollars is based on an agreed exchange rate as the quote was provided in Korean Wons (₩4,569,870)



3. Can you revise the quote to include insurance coverage. A simple one sentence statement at the bottom of the quote will suffice.



4. As a new supplier to SRI, please complete the following attached documents to your best ability :



a. Vendor cert form



b. FCPA certification form. This form is required by all foreign suppliers.
